Brookfield Centre

What do I need to know about the Brookfield Centre?

Brookfield provides a 30-bedded care unit for people who don't need to remain in hospital yet are not quite fit to return home; this could be for a variety of reasons. We also accept admissions from Urgent Care Response to prevent unnecessary hospital admission.

At Brookfield we provide intermediate care to promote independence. The following service professionals are available to support you, and will work with you to help you regain and retain your independence:

  • Intermediate care support workers
  • Physiotherapist
  • Occupational therapists
  • Advanced Nurse Practitioners
  • Community psychiatric nurse
  • Social workers
  • GP
     

When would I be placed there?

Sometimes when people are ready to be discharged from hospital, they may not be fit enough to go home straight away, or they may have difficulties managing at home due to illness or reduced mobility. Some may require a further period of recovery and/or rehabilitation.

Brookfield Centre is an Intermediate Care Unit where a person's needs are assessed and treatment is given in order for them to be discharged home safely. We aim to review your needs within seven days of admission.

All referrals to the Brookfield Centre are made through our Contact Cares team.

What can I expect? 

The multi-disciplinary team meet daily to discuss your care and to ensure you are receiving all the care you need while at Brookfield.

How will my care be managed?

  • Each service user has a care plan, which is drawn up in co-operation with you, and your family, and members of the Multi Disciplinary Team (MDT)
  • Plans are reviewed daily at the board round and discussed in more detail at the MDT meeting each week.

How long will I be there?

This depends on your needs following assessment. Once your assessed needs have been met we will plan with you for you to be discharged. You can continue your rehabilitation at home or an alternative community setting that provides a lower level of care. We will start to discuss your discharge plans early on during your stay, and you and your family/carers will be involved in the whole process.

What is expected of me?

  • We expect you to work in partnership with the Brookfield team to ensure you achieve your full potential
  • Please bring in your own clothing, toiletries and pads if needed
  • We ask that you participate in all assessments needed to establish a safe discharge
  • We ask that you do not keep any concerns, worries or issues to yourself. We are here to help so if you have any issues or concerns, please tell a member of staff as soon as possible
  • Smoking and the use of alcohol is prohibited at all council properties
  • We ask that you don't bring cash or valuables with you as we can't guarantee the safe keeping of these

Are there any charges for my care?

  • If your stay is longer than 42 nights, a financial assessment will begin.
  • Charges will be incurred if your stay is longer than 42 nights with no rehabilitation goals

What do I need to bring with me?

  • Toiletries
  • Nightwear
  • Own medications or TTOs (admission will be delayed if these are not included)
  • Your latest repeat prescription
  • Original Lilac form
  • Recent discharge from hospital letter
  • Any pending appointment letters
  • Books, iPad, magazines etc
  • Hearing aid, dentures, spectacles
  • Good fitting comfortable slippers/shoes
  • 2 x comfortable day time outfits
  • Mobility equipment eg. walking stick, zimmer frame, wheelchair

How can I or my family contact Brookfield?

Next of Kin

  • We ask that just one of your relatives or friends is listed as your next of kin and that we have their correct contact details.
  • We ask that only one member of your family should speak to us on a regular basis for updates.
  • A password system can be organised for telephone contacts. This enables the team to be confident that we are discussing your care with your full consent, only with your chosen contact.

Accessibility

All our facilities are at ground level and fully accessible. If you require any additional assistance or support, please speak to a member of the team.

Visiting Times

  • 1.30pm - 4.30pm 
  • 6.30pm - 8pm

More flexible visiting can be requested if required and extended visiting will be arranged on Bank Holidays.

All visitors need to report to reception at the front of the building, where they will sign in and out.

Parking is situated at the rear of the building.
